Affordable Care Management is the care management extension of our 15 year old MSO. Our MSO supports all of the necessary services for over 175,000 managed care lives. Affordable Care Management greatly improves upon the already stellar outcomes of our MSO.
Our Care Management Program was built from the bottom up to be the best in class coordinated care management service for your entire patient population. In addition, we meet or exceed the management, system and reporting requirements for the new Medicare/Medical Duals programs and offer a service specific to these difficult patients as a carve out offering.
